The following staff hold many years of experience and expertise which adds to the quality of care from Florida Hospital HospiceCare.

Kathleen Moore, LCSW, Assistant Director of Operations

In the health care field for over thirty years, Kathleen was the first hospice social worker in the Volusia and Flagler County area.  In her role as Assistant Director of Operations, she will primarily responsible for the strategic growth and development of the HospiceCare program.  Kathleen is also a Certified Grief Therapist.

 

 


Scott Baker, RN, Assistant Director of Clinical Care

A long time resident of Ormond Beach, Scott Baker joins HospiceCare as our Assistant Director of Clinical Care.  A Veteran of the USAF, he graduated from DBCC in 1993 with his RN degree.  In the Florida Hospital System since 1992, Scott comes to us as the former Director of Case Management with Florida Hospital Fish Memorial.  He will be responsible for the day to day clinical operation of HospiceCare.

Margaret Ann (Peggy) Smith, RN, BSN, Inpatient Coordinator

With Florida Hospital HospiceCare since November 2006, Peggy is in charge of both the HospiceCare at Oceanside Inpatient Unit and the Stuart F. Meyer Hospice House.

Raeanne Cade, Coordinator of Volunteer Services

Joining our staff in 2008, Raeanne comes to us with a wealth of experience in volunteer management.  At Loma Linda University Medical Center in California, she managed almost 1800 volunteers and is experienced in creating and coordinating progams and services that will benefit hospice patients and their families.

Darlene Kimble, RN, Clinical Coordinator Team A

With 13 years hospice experience, Darlene acts as coordinator of patient care working closely with the HospiceCare team to ensure that patients’ goals of physical, emotional and spiritual comfort are being met.

Leah Riddick, R.N., B.S.N., M.C.Ed, Performance Improvement Coordinator

Leah has come to the organization with over 25 years in nursing. She brings a wealth of experience from a diverse nursing background of ambulatory care, clinical care and has served over 8 years in the United States Navy in the Nurse Corps.  During her nursing career she has held positions as a utilization review coordinator, special projects coordinator and regional clinical nurse manager.  She holds a BSN from University of Central Florida and her Masters in Christian Education is from Dayspring Theological Seminary. Leah is also an Ordained Minister.

Holly H. Van Hoose, RN, CHPN, Admissions and Community Education Coordinator

With over twenty years of hospice experience in our area, Holly started as a hospice volunteer in 1983. Holly joins us from Hospice of Volusia/Flagler where she was the Admissions Coordinator for the past three years.  Holly has been a resident of Ormond Beach for 28 years and brings with her a passion for hospice.
