Where can I get hospice care?

While HospiceCare serves most patients in their own private home, services can be provided in a variety of settings.  No matter the location, the hospice team works to best serve the patient and family and to provide the best quality care.

At Home: For patients who choose to stay in their own home, we will do whatever possible to provide comfort care and keep them home during their final months of life.  This is the primary goal of hospice care. Visit Programs to learn more about hospice services provided.

At Alternative Home Sites:  For those patients residing in a nursing facility, long-term care facility, assisted living, group home or other residential care facility, HospiceCare provides the same quality care and considers the location the patient resides in as “home.”   We work closely with facility staff to coordinate the appropriate plan of care.

At the Hospital:  HospiceCare inpatient care can begin prior to discharge from a Florida hospital facility.  Equipment, medications and care needs can be established and prepared for the patient and family for continuity and quality care for the patient and a seamless transition “back home”. 

In our Inpatient Unit at Oceanside: There are times when a higher level of care is needed for controlling a patient’s pain or symptom management.  On these occasions, patients can be transported to our inpatient unit, HospiceCare at Oceanside.

At the Stuart F. Meyer Hospice House:  Located on the campus of Florida Hospital Flagler, the Stuart F. Meyer Hospice House is designed to provide care for those patients in crisis.  

Continuous Care: Continuous 24-hour support can be provided by professional hospice staff for those patients under hospice care in their own home as needed to meet a short-term critical care need.

Respite Care: Respite care may be available for your loved one, but this service is limited.  Contact our administrative office to see if you qualify.
